Admissions Director
Columbus Colony Elderly Care is now considering applicants for a talented and proven Admissions Director. Columbus Colony offers a full range of short-term rehabilitative and therapy services, as well as long-term skilled nursing care for up to 150 residents. It was established by The Ohio School for the Deaf Alumni Association, which opened the first Ohio Home for the Aged and Deaf in 1896. Today, Columbus Colony Elderly Care and Columbus Colony Housing, an independent living apartment building, form the foundation for a deaf-friendly campus providing housing, healthcare and recreational services to older adults in the Ohio.
Key Qualifications & Key Tasks:
- Ensure that every referral is processed and reviewed while providing a smooth transition from the hospital, community or any other setting.
- Responsible to establish referral sources and to obtain required information and admit residents in an efficient manner.
- Build relationships with area case managers, physicians, social workers, community organizations and hospitals to increase census and highlight the quality of care and services.
- Maintains the confidentiality of all resident care information
- Complete documentation and admission sign in paperwork for all new admissions, readmissions, and the completion of transfer notices.
- Develop and maintain business relationships with key referral sources and educate local community about our facility healthcare services
- Responsible for developing census and quality mix by providing prospective residents from acute hospitals, managed care provider case managers, assisted living facilities, physician groups, and senior communities
- Provide exceptional customer service as a liaison or coordination of services for resident referrals
- Develop and initiate marketing strategies that will increase facility census
- Participation on community and healthcare expos, health fairs, and other marketing events
- Other items as assigned as needed by the Administrator or Director of Nursing.
Requirements:
- Ideal candidate must be a self-starter, skilled communicator and positive motivator.
- Experience in SNF or senior care industry minimum of three (3) years’ experience.
- A nurse (LPN/RN) is preferred, but not required
- Must meet pre-employment requirements as outlined by the Ohio Department of Health
