LeadingAge Ohio is a nonprofit trade association that represents approximately 400 long-term care organizations and hospices, as well as those providing ancillary health care and housing services, in more than 150 Ohio towns and cities. The continuum of care reflected by the member organizations serve an estimated 400,000 elderly Ohioans annually and employ more than 35,000 persons statewide.

CMS Innovation Center Launches Guiding an Improved Dementia Experience Model, Announces Participants
Nearly 400 organizations, including two LeadingAge Ohio members of 11 total in Ohio, were chosen to test a new model aimed to improve the quality of life for people living with dementia, reduce strain on unpaid caregivers, and help people remain in their homes and communities through a package of care coordination and management, caregiver education and support, and respite services.
