MDS 3.0 Item Sets (v)1.20.1v3 and Item Matrix Finalized

The new Minimum Data Set (MDS) 3.0 Item Sets (v)1.20.1v3 and accompanying Item Matrix are now finalized and available in the Downloads section of the MDS 3.0 Resident Assessment Instrument (RAI) Manual page on the CMS site. This version contains changes finalized in the Fiscal Year 2026 Skilled Nursing Facilities (SNF) Prospective Payment System (PPS) rule and will be effective beginning October 1, 2025.
