You Asked: 

Is a resident’s DNR request valid if the nurse calls the physician, the physician gives a verbal order for the DNR, and the signed form is pending? Does the DNR go into effect during that time?

We Answered: 

According to the Ohio Department of Health (ODH), a DNR order is not valid or effective until the appropriate DNR form is completed and signed by a physician, Advanced Practice Registered Nurse (APRN) or Physician Assistant (PA). A verbal order alone cannot and should not be honored. The signed form is what makes the DNR directive actionable. Facilities must ensure that documentation is in place before recognizing or acting upon a resident’s DNR status.
