In last month’s meeting with the Ohio Department of Medicaid (ODM), ODM staff shared that they would soon release a standard form for providers and plans to use when sharing information about an individual, with a second part which includes detail about the individual’s past substance use disorder (SUD) if any. While the form was a requirement for ODM under the Ohio Revised Code, it is optional for providers. If an entity chooses to use the form, however, it must be accepted by the the other party. ODM anticipates certain segments of the provider community may adopt the form more rapidly than others.
The provider page on ODM’s website now contains information about the Standard Authorization Form, and ODM has also released a fact sheet on using the form.
