New process for self-reported incident reporting

On Thursday November 17, the Ohio Department of Health (ODH) sent out an Electronic Information Dissemination & Collection (EIDC) bulletin alerting providers to a change in the self-reported incident (SRI) process. ODH notes that in an effort to make SRI submission more streamlined for providers, ODH has updated the submission directions for the provider to submit an attachment with the initial SRI. 

Providers will no longer e-mail the SRI Initial Form to ODH. Providers are required to upload the SRI Initial Form directly into EIDC by following the attached instructions. Providers will complete the SRI Final form that includes all the new Centers for Medicare & Medicaid Services (CMS) required information for follow-up reporting. The fields on the SRI reporting in EIDC are currently limited so providers will need to complete the SRI Final form. Both the final report in EIDC and this form need to be completed with every initial SRI.
